Patient Care Coordinator
Must be a nurse - either LPN or RN.
Will be responsible for Discharge Planning, Case Management, Swing Bed Coordination and Utilization Review in a busy 25 bed Critical Access Hospital.
Assesses patient needs and makes arrangements for any medical equipment, home care services, or follow-up appointments that may be required for a patient upon discharge. Counsels caregivers and patients on proper handling of medical equipment being issued to their patients. Works with insurance carriers to ensure coverage of all medical equipment and post-discharge services that may be required. Works with patient and clinic to ensure proper after discharge care and appropriate hand-offs for best short-term outcomes and long term health status of the patient. Conducts post discharge phone calls to patient within 48 hours of discharge.
Develops and implements discharge plans by assessing the patient’s psycho social needs. Provides social services to patients and families. Counsels and educates patients and families regarding therapies and treatments. Reviews and modifies discharge plans as needed. Makes patient and family referrals to community support services as appropriate. Collaborates with other healthcare and management personnel to provide optimal family centered care. Engages and educates medical staff and hospital staff as necessary regarding the Discharge Plan.
Assesses and evaluates patients to ensure progress towards desired outcomes. Responds to patient/family needs prioritizing patient and family preferences. Rounds with physicians as well as attends morning report with nursing staff. Coordinates with physicians and other clinical staff regarding the patient’s/family's preferences, patient progress and discharge needs.
Swing Bed Coordination
Owns and manages the entire Swing Bed process from start to finish which includes ensuring the appropriate orders are in the EMR and signed. Responsible for notification of Insurance as well as follow up post authorization. Screens swing bed patient admissions to ensure proper placement into available beds, while adhering to hospital policies and procedures. Performs swing bed admission and discharge assessments and ensures all regulatory requirements are met. Coordinates and schedules swing bed care conferences for all swing bed patients and their families. Periodically reviews swing bed admission procedures and suggests changes to improve the overall quality of care.
Conducts prospective, concurrent and retrospective review for medical necessity and the utilization of ancillary services to ensure the appropriate level of patient care. Determines appropriate level of care through collaboration with physicians. Reviews, monitors, evaluates, and coordinates patient's stay to assure services are timely and efficient. Maintains appropriate documentation. Confirms compliance with federal, state, and local requirements, as well as organizational policies and procedures.
Wage Range $16.25 - 28.65 - DOE
